The self driving car should theoretically be seeing and predicting any eventuality. It uses sensors to see what the human eye cannot. So theoretically, it could see the boxes becoming more unstable within the truck and therefore take corrective measures (like slowing down and giving more distance to the vehicle in front) before the boxes unload.

Here in Ontario they're pushing something called the Dutch Reach. Encouraging drivers to open their door with their right hand, forcing them to turn their head and see if anything is coming as they open their door. Simple stuff like this goes a long way.
