Competent investigators would not reach conclusions based upon this video alone. Other video of the crash scene at the same time of day shows the lighting to be much brighter. There would also be on site inspection of the physical surroundings (including the lighting), the vehicle (including all equipment and recorded data), the driver, and evidence from the crash scene.
Humans are going to act in unpredictable ways. Automatous vehicles will have to correctly react to those actions all the time, every time. I don’t disagree that there appear to be several layers of failure in this incident. Many incidents result from the compounding of seemingly small errors. Early hypotheses may prove to be correct, but I prefer to hear the full explanation and conclusions of the experts, as in the case of past catastrophic events.
