I made ChatGPT freely admit it had suggested replacing bromide with chloride with one question. It in fact offered to quote the exact scientific journal where this happened with all relevant details. I think in your case it got defensive because you accused it directly and also followed this up with severe consequences for the affected person, directly implicating it in harming people.
