People with severe untreated malignant narrcissitic personality disorders often live in a "fantasy world" and have stunted emotional developement, so to speak. They do not ever understand or accept consequences they are incapable of it. Trump could nuke Iran and it doesnt matter the consequences to him as he will automatically envelope himself in his fantasy world where he is the good guy or the hero. This is why this man is and has been a threat to the world.
