We can safely say that the real number was much higher, but even putting it at 10 times higher (imho this is stretching it, debate away) up to 680k, this is only 1% of the population of Hubei. So yeah, no wonder that no herd immunity is found if the contamination rate was somewhere between 0.1-1% of the population. -- Edit: although the OP says "less than 4%", so my own estimate might be off as well. I guess if we take Wuhan's population instead of Hubei's, we're up to 5% contamination rate more or less.
