I think the idea is, care is needed in parts of the world where people can't afford it (due to circumstances outside of their control). Without care, the impoverished nations of the world fall even further away from the goals of health and self sufficiency, so the nations that *can* afford to help, do so in order to save lives and help the global community, rather than just hoarding all the money within the borders watching humanity suffer outside. Also, $600 million is nothing to the federal budget. That amount is covered completely by this year's BONUSES to a handful of Trump's cabinet members alone. So it's very little to the USA, but means a LOT of lives improved elsewhere, why not give where we can? The "you're not me, so why would I help you?" mentality is a bad way to do things when you're the biggest power in the world.
