It's going to get more complicated but fortunately more complicated systems do exist and investigations are already carried out in a very different manner, I am talking about the aviation industry because airplanes themselves are such complicated machines and operate within a complicated systems that are managed by a large number of people at the same time so if an accident happens you cannot directly point fingers at the pilot like we normally do or think we should do instead each and every detail of the reason for the accident is investigated and recommendations are brought out that include changes in the avionics, Changes in management, changes in training, just to mention a few ...... With all of these being done it makes the flying of millions of unsuspecting commoners accross the world safe.....

Why I mentioned it? Because that's where we're headed with driverless cars there would be people sitting in RTC or road traffic control rooms who will issue traffic commands to the vehicle, if any accident occurrs it can be due to all the factors you mentioned and all of them will be investigated to see what needs to be changed
