There has been cases of people mixing up the accelerator and the brakes and causing accidents for all long as we've had automatic transmission cars.
The NHTSA investigated these claims for Teslas and found zero faults. It's always been a result of user error.
Look up "unintended acceleration event" on Wikipedia. Long history of this type of accident.
By the way, the car cannot keep you from braking.
