I don't like that people call this sort of error "hallucinations". It's a lousy analogy. LLMs are sophisticated mimic, the have no understanding. Their mimicry just drifts away from reality. It isn't a halluncination which is a person seeing something distorted or that is not there at all. It has nothing to do with seeing. The LLMs just make stuff up.
