This result was expected to be honest. Pretty much have to ground everything an LLM does by having it research best practices. Can never just ask or this happens :/. Research can get tricky because you have to manage potential gaps and misses and detect when you should iterate with other focuses. There are a lot of failure points. You end up writing thousands of lines of markdown for workflows to reign in bad behaviors.
