AI has been the cause of hospitalization for 2 patients from my pharmacy alone within the past few months. One patient was in the hospital for a full month. Another AI error gave a patient a grossly neglegent quantity of a controlled substance and we only knew bc the dr called wanting to know why the wrote for 1 tablet and we gave them a months worth w refills. So much of our system is AI now its very likely that far more errors are making it out the door. This is just over the past few months at one of the US's largest retail chains, at just one location. This doesnt include our insurance finder cross billing patients.
