I wouldn't be surprised if the access door didn't have a safety interlock, or if it did then it was bypassed. Generally, it'd be tied back to the safety controller (or safety relays) which would open the fence circuit on the robot unit. The fence circuit can be bypassed by physical means (jumpers in the control cabinet), which may be more common to do in their facility. Either way, someone didn't do proper safety commissioning on the cell or failed to do routine safety circuit checks.
