I don't think it is possible to completely eliminate hallucinations because it is based on probability and that can't be 100% unless you include the whole solution set. Like for example there is a 100% chance it will rain or not rain but there can't be a 100% chance it will rain. I think that unless AI is capable of true reasoning it won't be able to understand with certainty what is correct.
