Trying to make the scenario above perfect would mean the boxes have to be fastened by robots so they do not fall off and if they fall off they don't roll to the other lane and if they roll to the other lane the self driving car on that lane would have given enough braking distance because it had preempted the fall of the boxes. Truth is as long as humans design, maintain and use self driving cars we would still have accidents. My computer crashes cos I'm human and the designer is human.
