The accountability gap he's describing isn't new to AI. Large organizations solved the same problem decades ago. You distribute decisions across enough layers that no single person made the call. The AI version is just faster and has better PR. The structure is identical: capability advances, accountability stays theoretical, and when something goes wrong there's no one to point at.
